Keep Your Dog Healthy With Routine Health Care

As a dog owner you are responsible to keep your dog healthy with routine health care. This involves so much more that just regular feeding and exercise. Much of a dog’s care can depend on its age and general health. It can also be driven by your work or social schedule. Regardless, you should keep the following in mind to help

ensure that your dog lives a happy, healthy and long life. So, in no particular order:

  • Be watchful– Dog are actually great communicators, if you pay attention. This is true from “puppyhood” through to their elder years.
You will come to know their demeanor at various times of the day. Changes in this can be signaling a change in health. Watch for reduced energy, a lackluster coat, excessive weight gain or loss, unusual growths, restlessness/anxiety or even the loss of the usual spark in the eye. If you are a dog owner already, you will know what I mean. In all cases, seek the advice of your dog’s healthcare professional.
  • Establish a daily routine– Dog’s love routine. It is through this routine that they come to understand their role in you family (read Pack). Create
a morning, daytime and evening series of events that your dog will respect and look forward to such as:

    • Morning walks and light playtime.
    • Feeding – Depending on your pet’s age and health, daily feeding times and amounts do vary. In all cases, ensure that the feedings are earned with a simple wait command for a brief moment prior to allowing the feeding to begin. Some animal behaviorists argue that your pet should never be feed before you and your family, re-enforcing the Pack process in the wild.
    • Potty times. Again this will depend on the age and health of your pet. Generally, our experience is that a puppy will need a break one hour for its monthly age. For example, a two month old puppy will need a break approximately every two hours; a three month old will require a break every three hours, and so on. Adult dogs will usually need approximately 2-3 breaks per day depending on time of year, food and water intake, etc.
    • Evening Playtime – Your dog will want to play with you especially if you have been absent through the day. Establish a playtime/routine that will convey to your dog that this is a time to be happily anticipated. Also, to help with the understanding that there is also a downtime required so that you may get on with your other obligations or interests at home.
    • Bedtime On Time – Just like humans, bedtime at a regular time in the evening is preferred. Have your pet either kenneled or directed to its “lay down” space at approximately the same time each night. Ensure that they have done their business and have not been overly excited prior to this time.
  • Kenneling– This is becoming the much preferred way for your pet to be trained and be housed in doors. It has been our experience that a dog will, if introduced to a kennel properly, heartedly accept it as a secure, comfortable place to occupy. Allow your pet to run the house especially as a pup or younger dog will make house training more difficult. The kennel represents a positive place for your to put your dog between feeding/potty trip early on and also a place for an older dog to rest, sleep or just plain chill. The size of the kennel is very important and dependent on your pet’s size. Seek the advice of your sales clerk prior to purchase.
  • Choose healthy, age appropriate dog food– This is so important for your dog. The adage that we are what we eat applies to your pet as well. Take some time to read the label of various dog food bags at your place of purchase. You will be shocked at the ingredients found in the offerings. Price is not an indicator of healthy food either. Obviously, natural ingredients are the best. The better the food intake, the better your dog will look, feel and act. This is very important folks!
  • Exercise your dog – Exercise yourself– Your pet is going to need exercise. Some breeds admittedly require less exercise but generally all dogs need some level of exercise. Beyond the physical benefits of the routine, don’t underestimate the psychological benefits as well. A dog without exercise is going to become frustrated, anxious or even aggressive at times. Also, each time you walk or play with your pet you are increasing the bond between you. This is emotionally beneficial to both of you. Lastly, each step you take during this routine is a step closer to good health for you. Vary the routine if you can. It will be more enjoyable and interesting for your dog.
  • Toys should be age appropriate– Toys that are not age appropriate can actually be dangerous for your dog. Certain types of rawhide bones could, after being chewed by a puppy, lodge in its throat and be quite a threat. Dogs at each age plateau have unique needs and abilities with respect to toys. They can help to stimulate, educate, develop structurally and amuse your pet. The use of toys are also important. It has been suggested, for example, that playing tug of war with your pup and letting them tug the toy from you may be sending it the wrong message. It may learn that it can beat your or dominate that particular exercise. Check with your Vet for advice in this area.
  • Prevention save money and extends life– Regular checkups with your Vet are absolutely essential to the quality of life for your dog. Beyond the physical assessment that will be taken, this is a good opportunity for the following:
    • Update vaccinations and apply boosters if necessary
    • Treatments for tick, heartworm and fleas may be applied
    • Tests for bloodworm and fecal evaluation may be done

If there is a problem, chances are it will be diagnosed, treated and resolved quick saving you pet unnecessary discomfort, trauma, or a more serious outcome down the road.

  • Grooming is more that just looking good– Regular grooming is important on a number of levels. You want your dog to look good and this will do the trick. If you decide to do this yourself, it is yet another chance to increase the bond that you have with it. In addition, the groomer may be able to spot any unusual skin ailments that can be brought to your attention and assessed by your Vet. Grooming for your pet can be as anticipated as a long awaited massage for you. Most dogs will love the attention. This is also a time to trim nails. This is important to your dog’s health. Not trimming them properly or very infrequently can hurt your pet or actually affect its ability to run, walk or remain stable on certain types of flooring. Regular trimming alleviates this.

Just a few ideas that will help you to keep your dog healthy with routine health care. It takes some work and attention but the benefits to your dog are immense. This is your companion after all. Do all you can to provide it with a long, healthy and happy life.

Simple Ways to Transform You Health

In Power Tools to Success, the podcast episode, we spoke to Ronald Abvajee who is a Health and Fitness Expert to ask him how you can transform your health and fitness in as little as ten days. The subject of health and fitness is not an easy one, there too many contradicting advises for diet and so are exercise programs. In life you succeed when you do what you love and enjoy. The strategies we share with you are meant to help you start with fitness and exercise from a place of enjoyment. Only after you start enjoying fitness and exercise can you get into serious exercise programs. The truth is there are simple ways to transform your health and fitness that you can begin to use today, immediately. Here are the few of them:

  • Do not eat for taste, eat for sustenance. Eating for taste leads you to eat not only unhealthy meals but larger meals too. So try to eat for energy so that you are able to get on with your day and carry your work every day.
  • Get up from the couch every day, walk, take a walk with your kids, or walk your dogs or walk with your partner. A simple walk every day can lead to burning of calories up to 250 and goes along way towards your health and fitness goals. You do not have to stick to walking perhaps you can do what you like, play tennis with a friend if you want to or run or play touch rugby. Whatever gets you off the couch and gets you physical, do it. The more fun the better.
  • Avoid sugar drinks. Rather, drink raw fruits. Invest in a juicer. Drink a lot of water too.
  • Vegetarians stand a better chance of losing weight than meat eaters. You don’t have to become a vegetarian but you can go without meat for a week or two and watch see if doing so helps with you weight. For most people, going without meat a few days on their diet helps. When not eating meat most people eat less than they normally would. This can help a great deal towards your goal of losing weight.

The above strategies will help you improve your health and fitness in as little as ten days. Once motivated enough, you can then start a formal exercise and diet program to help you reach your fitness goals.

Why Does My Health Insurance Want Me To Fill Out A Form After I’ve Been Injured In An Accident?

You have been sent the form because your health insurance has paid for medical bills which it believes may be the result of someone else’s fault. If so, they want to find out that information because they may have a right to be repaid or reimbursed if you obtain a settlement or recovery against the at-fault person, company or insurer for that at-fault person or company. This is called “subrogation”. Your health insurance policy will probably have a “subrogation” section or clause stating this.

It is also in your best interest to complete and return the form to your insurer because if you do not and you get a settlement or recovery in your personal injury case, your company may come after you for the amounts they’ve paid.

Under Pennsylvania Law, when you are injured in an accident, you may have to submit your medical bills to your health insurance. You may be wondering why you have to do that if the injury was caused by someone else and they have liability insurance. For example, If you are injured due to falling or being injured as a result of a dangerous condition to property, or as a result of an unsafe product, unless the liability insurance company has “med pay”, you have to submit your medical bills to your insurance. Your insurance may have a right of subrogation, meaning if you obtain a settlement or recovery against a third-party, your health insurance has the right to be repaid.

So, in the end, the at-fault insurance company is paying the medical bills by being responsible to reimburse your health insurance company from a settlement. Your health insurance is simply fronting it and the form they want you to fill out facilitates that process.

Health Risks of PVC Roofing Membrane

Many building owners and manufacturers have asked me why I don’t offer PVC membrane to my flat roof customers. I simply feel that PVC is too great of a risk to take.

What is PVC?

PVC, or Polyvinyl Chloride, is made from the polymerization of vinyl chloride monomer (VCM), a toxic, carcinogenic gas. It is a durable, economical material with many uses. 75% of all PVC consumption in North America is attributed to plastic pipes. PVC is also used as a membrane to cover flat roofs, and appears to the average person to look exactly like TPO membrane.

What are the risks associated with PVC?

Research shows that dioxin, which is the most potent carcinogen known to science, along with vinyl chloride and certain acids, are created and released during the production of PVC. These chemicals can cause severe health problems such as:

  • cancer
  • neurological damage
  • immune system damage
  • respiratory problems
  • liver and kidney failure
  • birth defects
  • asthma

These are problems that the public and roofing contractors have a right to know about.

My story

About 12 years ago, PVC was the primary material we used in our flat roofing division here at Brady Roofing. After we began installing PVC, I started to experience health problems. I have been healthy my entire life, so when these issues came on so suddenly, it made me wonder about the safety of PVC membrane. This led me to do some serious research on the material. I found that manufacturers actually know that PVC membrane is very toxic, but they were hiding these facts from me during our conversations.

My personal health problems included severe respiratory issues and neurological damage. The neurological issues are actually getting worse, despite the fact that I no longer install PVC. I don’t want to go into my own health problems too deeply, but just to illustrate, my left lower body is experiencing a slow atrophy of the muscles due to neurological damage. My left calf is currently 1.25 inches smaller in circumference than my right. While I have no proof, I strongly believe it is caused from breathing the toxins found in PVC membrane. I have no genetic predisposition for such neurological issues.

Is this research relevant?

I approached our PVC manufacturer about the issue. They simply told me that there was no research to support my claims. In fact, there is a great deal of research done on the toxicity of PVC. The manufacturers can claim they are not relevant because these studies are done on the production of PVC rather than the installation risks. However, when you install PVC, you use hot air up to 1300 degrees F to melt the membrane and seal it. As the membrane melts, it can feel like you are breathing ammonia. The air can burn your sinuses, showing the presence of harmful chemicals being released, just as they are during production. Manufacturers will not provide this information to contractors and consumers.

Their argument is that the roofs are installed in open air, so there is enough ventilation to keep the installers safe. However, when you are welding, your face is 18 inches from the fumes. In a large majority of flat roofs there are also parapet walls, keeping the wind from blowing and clearing the fumes out. A significant percentage of the time, the fumes will enter your sinuses instead.

What can we do?

All of my research led me to believe that PVC is not a safe material to handle. This is why we have refused to install PVC membranes. Though there are variations of PVC membranes created in an effort to distance themselves from the hazards associated with PVC, careful examination will reveal that vinyl chloride monomers and dioxins are still present in these alternatives. While I have not drawn any strict correlations between PVC health risks and the customers, I have drawn risks to the installers, and I refuse to take any possible risk with our customers health as well.

We have chosen a new, innovative roofing alternative called TPO. TPO looks identical to PVC, but it comes without risks to the installer or customer. I believe that it is actually a better membrane because it does not get brittle like a dried up PVC pipe, as is characteristic of PVC sheets. TPO is flexible and highly energy efficient, along with being non-toxic. TPO is a younger product, it has only been around since 1991. However, it is showing indications that it will last longer than PVC membrane.

Five Tips for Choosing the Right Health Insurance Plan

One of the best ways of safeguarding your personal well-being is by having a health insurance plan. Due to the large number of companies that offer this type of insurance policy, choosing the right one can be a daunting task. Here are five tips to help you find the best plan on the market today.

Check the Networks of Different Plans

If you have a select group of doctors whom you prefer, ask them which insurance networks they are part of. You can also use one of the many accredited online directories to know the networks that each plan on your list has. On the other hand, if you just relocated to a new city and you do not have a preferred doctor yet, choose a plan that has a large network of health practitioners.

Know the Maximum Premium Budget

Consider your average monthly expenses to know the amount of money that you can spend on the policy without compromising your financial capability. Low premiums often result in high out-of-pocket costs. Hence, what appears to be the cheapest plan may really be the most expensive, as you will be forced to pay for most of the services. One of the best ways of choosing the right premium is by estimating the total amount of money that you spent on health care last year, including out-of-pocket expenses and premiums. Then, choose a plan that has similar monthly deductibles, copays, and premiums. A high premium plan will save you thousands of dollars on out-of-pocket costs if you require expensive procedures or frequent medical care services.

Look for Additional Benefits

In a bid to attract more clients, insurance companies usually offer policies that have additional benefits apart from medical coverage. Based on this fact, it is wise to check the summary of benefits of each policy on your list until you find one that has your preferred scope of services. For instance, some policies provide better coverage for mental health and physical therapy care while others have more comprehensive emergency coverage.

Research Plan Structure

Once you have an estimate of your monthly premium, your next step is determining your ideal plan structure. Some of the most common plans are HMO, EPO, POS, and PPO. The plan that you select will determine which doctors you can see and your costs. More often than not, plans that have referrals, such as POS and HMO, require that you first consult a primary care physician before booking an appointment with a specialist. If you already have a network of doctors that you prefer to see, eschew plans that they do not take.

Compare Out-of-Pocket Costs

Your preferred health insurance plan should clearly indicate the services that you will be required to pay for and the amount. The sum amount includes coinsurance, co-pay, and deductible. A plan that has low out-of-pocket costs is an ideal choice if you regularly need emergency services, take expensive medications, have scheduled upcoming surgery, or were recently diagnosed with a chronic condition such as cancer or diabetes.

Finally, make sure that the health insurance company is legally accredited, reputable, and reliable to cushion yourself from any inconveniences.

The Lost Costs With Administrative-Related Tasks With Group Health Plans

Health coverage is expensive- both for individuals and for companies that provide it.

The costs affect much of the medical field, including drug prices, cost of coverage,

costs of care and visits, and a myriad of other areas of the health industry. Part of

those costs is resulting from the administrative handling of health insurance logistics,

and those costs affect the rest of the field, too.

According to studies in the field, noted by the CAQH Index, in 2019 they noted that

“SPENDING ON HEALTHCARE ADMINISTRATION COSTS AN ESTIMATED

$350 BILLION ANNUALLY IN THE UNITED STATES DUE TO IT’S

COMPLEXITY.”

Data from the 2019 CAQH Index indicates that $40.6 billion or 12 percent of the

$350 billion spent on administrative complexity, is associated with conducting

administrative transactions tracked by the CAQH Index. Of the $40.6 billion spent on

these transactions, $13.3 billion or 33 percent of existing annual spending on

administrative transactions could be saved by completing the transition from manual

and partially electronic processing to fully electronic processing. The progress that

the industry has already made to automate these administrative transactions has

saved the industry over $102 billion annually.”

Administration is, of course, an important aspect of any industry, especially one as

complex as medical and related fields. The difficulty with modern health insurance

means extensive administrative hours as they tend to a myriad of issues on multiple

fronts. This means, as noted earlier, a great deal of expense that filters throughout

the medical field.

Unfortunately, small business owners tend to bear the brunt of these costs, at least

when it comes to businesses rather than people. As noted here,

“NOT SURPRISINGLY, THE COST OF PROVIDING HEALTH COVERAGE TO

EMPLOYEES LOOMS LARGER THE SMALLER THE BUSINESS,

BUT THIS ISSUE PLAGES BUSINESSES REGARDLESS OF SIZE”

The price tag on health insurance is a significant pain point for small employers. The

problem extends to recruiting and retaining talent, as well. To compete with larger

employers, small employers are hard-pressed to offer benefits like health insurance,

even as the benefit takes up a larger share of the bottom line. Two-thirds of

businesses (69%) said the problem has been getting worse. They reported that costs

have increased over the last four years; one-third of this group reported annual

increases of 10 percent or more. Businesses with fewer employees cited bigger

increases than larger businesses. Employers cited prescription drugs and lack of

choice of health care plans as pain points.

There are ways to curb this expense without impacting the medical field or health

insurance. One method is the increased use of digital materials. According to the

previously cited Index, “Although partially electronic transactions often cost less and

are less time consuming than manual transactions, there are savings opportunities

associated with moving from partially electronic web portals to fully electronic

transactions. For the medical industry, $2.7 billion of the $9.9 billion total savings

opportunity could be achieved by switching from partially electronic transactions to

fully electronic transactions. The greatest per transaction savings opportunity for

medical providers is a prior authorization. Medical providers could save $2.11 per prior authorization transaction by using the federally mandated electronic standard rather than a web portal. Understanding the impact of portal use in more detail is important as the industry focuses on opportunities to decrease administrative costs and burden.”

The medical field is one area where increased use of digital technology has lagged in

comparison to other fields. Concerns over confidentiality and security, combined with

outdated legislation, mean much in the medical field is handled with pen and paper.

That said, the COVID-19 pandemic has resulted in rapid inroads in digitization. Still,

administrative costs remain high, with subsequent effects throughout healthcare.

Along with the use of digital technology, another way to reduce costs is through increased automation. As noted by the previous study, “The 2019 CAQH Index estimates that the medical industry has avoided over $96 billion in annual administrative costs through efforts to automate administrative transactions. By comparison, the dental industry has avoided over $6 billion annually. For both industries, the largest annual savings has been achieved for eligibility and benefit verification at $68.8 billion for the medical industry and $3 billion for the dental industry. However, although the industry has already avoided significant administrative costs through automation, 33 percent of existing spending could be saved through further automation.

To continue to drive progress, harmonization is needed across all stakeholders to

reduce administrative costs and burdens. Aligning on a common understanding of the

barriers to electronic adoption and the business needs of the future is imperative for

plans, providers, vendors, standards development organizations, operating rule

authoring entities and government to maintain and improve upon industry

achievements to date.”

There are other ways to mitigate costs as well, without subsequent suffering in quality. One way is to reduce what one article sites as administrative waste. As noted by said

article,

“ADMINISTRATIVE WASTE AS ANY ADMINISTRATIVE SPENDING THAT

EXCEEDS THAT NECESSARY TO ACHIEVE THE OVERALL

GOALS OF THE ORGANIZATION OR THE SYSTEM AS A WHOLE.”

The National Academy of Medicine’s seminal 2010 work, The Healthcare Imperative:

Lowering Costs and Improving Outcomes, identified unnecessary administrative costs

as one of six key areas that need to be addressed to bring greater value and lower

costs to healthcare consumers.

ADMINISTRATIVE COSTS HAVE BEEN ESTIMATED TO REPRESENT 25-31%

OF TOTAL HEALTHCARE EXPENDITURES IN THE UNITED STATES,

a proportion twice that found in Canada and significantly greater than in all other

Organization for Economic Cooperation and Development member nations for which

such costs have been studied. Moreover, the rate of growth in administrative costs in

the U.S. has outpaced that of overall healthcare expenditures and is projected to

continue to increase without reforms to reduce administrative complexity.

It is thus important to differentiate administrative waste from necessary

administrative spending. As noted by the previously cited article, “A key segment of

wasteful administrative spending is found in the significant amount of paperwork

needed in our multi-payer healthcare financing system. Having myriad payers, each

with different payment and certification rules increases the complexity and

duplication of tasks related to billing and reimbursement activities. Hence,

“THE TOTAL BIR COMPONENT OF ADMINISTRATIVE SPENDING-

REPRESENTING ABOUT 18 PERCENT OF TOTAL HEALTHCARE

EXPENDITURES-IS OFTEN SINGLED OUT AS WASTEFUL AND A

POTENTIAL SOURCE OF SAVINGS. AN OFTEN-CITED STATISTIC IS THAT

HOSPITALS GENERALLY HAVE MORE BILLING SPECIALISTS THAN BEDS.”

A problem with separating administrative waste from proper administrative costs is

insufficient data. While healthcare provides, creates, and utilizes fast amounts of

data, that information is geared to specific fields and areas. As a result,

administrative data tends to be neglected and understudied. As this article notes,

“Our current understanding of administrative spending relies on a patchwork of

mostly aging analyses, leaving policymakers very much in the dark when it comes to

addressing this growing category of healthcare spending.

MOREOVER, PATIENT ADMINISTRATIVE BURDENS HAVE NEVER BEEN

TALLIED, REPRESENTING THE GREATEST GAP IN OUR UNDERSTANDING

OF ADMINISTRATIVE BURDEN. PATIENTS INCUR ADMINISTRATIVE COSTS

WHEN THEY ENROLL IN COVERAGE, RECEIVE CARE, AND GET

REIMBURSED FOR EXPENSES. PATIENTS WITH PARTICULARLY COMPLEX

NEEDS MAY EVEN RESORT TO HIRING A PATIENT- OR MEDICAL-BILLING

ADVOCATE OR AN ATTORNEY.

Other data gaps include research to identify potential administrative waste associated

with provider credentialing, pre-authorization or grievances and appeals.”

Though more data may be needed in regards to understanding administrative waste,

there are still methods to handle it and ensure expenditures on administration in

healthcare are spent properly. This will help reduce overall healthcare costs,

including health insurance. One of the costliest areas of administrative costs is

billing. This issue has been known for some time. As noted here, “In 2010, the ACA

tried to rein in administrative waste. In recognition of the high cost of billing and

payments, section 1104 of the ACA required the US Department of Health and human services to promulgate rules to standardize many aspects of billing and payments. Specifically, the ACA called for a national system to determine benefits eligibility, coverage information, patient cost-sharing to improve collections at the time of care, real-time claim status updates, auto adjudication standards, and real-time and

automated approval for referrals and prior authorizations. These actions were

supposed to be implemented in 3 waves in 2013, 2014, and 2016. However, only the

first 2 waves were implemented in 2013 and 2014. These regulations standardized

eligibility required real-time claims status, and created electronic fund transfer

standards.

THE MOST COST-SAVING ACTIONS, AUTO ADJUDICATION OF CLAIMS

AND PRIOR AUTHORIZATIONS, WERE SUPPOSED TO BE

IMPLEMENTED IN 2016 BUT WERE NEVER ENACTED.”

The matter is complicated by how to diffuse healthcare is within the United States.

There are federal administrations, state administrations, regional groups, corporate

groups, church groups, local clinics, and clinics operated by chains, such as CVS

Minute Clinics. The previously cited article makes note of this, stating that

“BECAUSE THE US HEALTHCARE SYSTEM IS SO FRAGMENTED, THERE

IS NOT A CLEARLY DOMINANT ENTITY TO SET ADMINISTRATIVE

STANDARDS AND FORCE ADOPTION.

The federal government is the largest payer, but its market power is not concentrated

because its payments flow through hundreds of different programs, including 50

unique Medicaid programs, Medicare, hundreds of Medicare Advantage plans, ACA

insurance exchanges, federal employee health benefits, the military health system,

Veterans Affairs, and the Indian Health Service. Each of these programs has governance over its administrative rules. Some programs, such as Covered California, use their local market power to force standardization of administrative elements, such as benefit design. The private sector alternatives lack either geographic reach or local market scale. The largest private sector entities are

the payers United Healthcare and Anthem. However, neither of these companies are

positioned to be administrative standard setters. United Healthcare lacks a local

market scale because it usually only accounts for 10% to 20% of patients for

clinicians. Anthem lacks geographic scale because it only operates in 23 states. Only

the Medicare system operates in all states and is accepted by nearly all health care

organizations, which means changes to Medicare’s administrative rules are adopted

nearly universally. Medicare is also a large payer, through the Medicare Advantage

program, to the largest commercial payers, which could enhance Medicare’s ability to

serve as an administrative standard setter. This makes Medicare the only participant

with the market power to set administrative standards.” As Medicare for All seems an unlikely, though useful solution,

OTHER AVENUES TO CURTAIL ADMINISTRATIVE WASTE NEED TO BE

CONSIDERED. ONE SUCH METHOD WOULD BE INCREASED USE OF

BILLING SPECIALISTS TO REDUCE THE NEED FOR ADMINISTRATIVE STAFF,

AND, AS A RESULT, THE AMOUNT OF ADMINISTRATIVE SPENDING.

Billing specialists are a good example because of the decentralized nature of the

United States healthcare systems. Centralized billing, even by a third party, would

help to reduce costs. As noted here, “Germany and Japan both have multiple payers

but centralized claims processing. Despite having more than 3,000 health plans,

Japan’s administrative expenditures were a stunningly low 1.6 percent of overall

health care costs in 2015, one of the lowest among OECD [Organization for Economic Co-operation and Development] member nations. In their analysis of three universal health care options for Vermont, including single-payer, researchers William C. Hsiao, Steven Kappel, and Jonathan Gruber estimated substantial savings from administrative simplicity from each option. The two single-payer options they examined would result in even greater administrative savings of between 7.3 percent and 7.8 percent, depending on the rate-setting mechanism. The group estimated that a third scenario, which would establish a centralized claims clearinghouse while allowing multiple payers, could generate savings equal to 3.6 percent of total expenditures. This suggests that about half of the total administrative savings from a single-payer system could be obtained within a regulated multipayer system.”

THUS, BILLING SPECIALISTS, ESPECIALLY OUTSOURCED SPECIALISTS,

CAN HEP REDUCE OVERALL HEALTHCARE COSTS.

As this article notes, “This process is more straightforward than in-house billing for

medical practice staff. They can scan and email superbills and other related

documents to the medical billing service provider.

Most medical billing service providers charge a specific percentage of the collected

claim amount, with the industry average being approximately 7 percent for

processing claims.

The convenience factor is a major reason that medical practices choose to outsource

their billing. A provider handles all the data entries and claim submissions on behalf

of the medical practice. They also follow up on rejected claims and even send invoices directly to patients.

If a medical practice is using electronic health records (EHR) software, then this

process becomes even easier. Practices can store information from a patient’s

superbill in the EHR and securely transfer data to the billing service provider using

the interoperability feature. This eliminates the need to manually scan and send

documents.”

There are benefits to in-house billing as well. The previously mentioned article

mentions that “The in-house billing procedure for processing insurance claims

involves many steps that are universal to every practice.

First, the medical staff enters information into the medical billing software from a

superbill that’s prepared during a patient’s visit. The superbill contains specific

diagnosis and treatment codes, along with additional patient information that the

insurance company needs to verify claims.

Using the software, the practice submits the claim to a medical billing clearinghouse,

which verifies the claim and sends it to the payer. The clearinghouse scrubs the claim

to check for and rectify errors (for a fee) before sending it to the payer. By not

submitting claims directly to a payer, the practice saves time and money and lowers

its claim rejection rate.”

BILLING SPECIALISTS, EITHER IN-HOUSE OR OUTSOURCED, ARE AN

EXCELLENT WAY TO REDUCE OVERALL HEALTHCARE COSTS.

By reducing administrative waste, costs, in general, can be reduced. This also means

those savings will, at least in theory, be transferred to clients. This is especially

important for small businesses, who are often the hardest hit when it comes to paying

for health insurance. As demonstrated, a major issue for health costs and their

increase is related to all the administrative costs.

Several studies have shown this to be true. As referenced in this article, “A new study

from Stanford University finds that

THE TIME EMPLOYEES SPEND WITH INSURANCE ADMINISTRATORS

CLEARING UP QUESTIONS AND ISSUES-CALLED “SLUDGE” BY

RESEARCHERS-HAS COSTS IN THE TENS OF BILLIONS ANNUALLY.

The study, led by Jeffrey Pfeffer, a researcher, and author found

THAT THE DIRECT SOTS OF TIME SPENT BY EMPLOYEES ON HEALTH

INSURANCE ADMINISTRATION WAS APPROXIMATELY $21.57 BILLION

PET NATURAL HEALTH CARE SOLUTIONS

Remove hair

Removing hair on and around the hot spot should be your first step. It is very important in order to let the skin breath, helping the hot spot to heal. Be sure to use a blunt end scissor such as nose hair scissors. Be as gentle as you can, because the hot spot area is painful and your pet will feel discomfort just from the hair being cut.

Clean the Wound

It is very important that you clean the wound by using a MILD, non perfumed antiseptic soap to clean. Use a cotton ball and gently dab the wound with the cleaner. Be sure to rinse well and not leave any soap residue behind.

Tea and Aspirin

The key to healing the hot spot is to get it to dry out and there are some very powerful natural aids that can do this and are probably already in your kitchen. One of them is black tea and the other is aspirin. Make a strong cup of black tea and dissolve an aspirin in it. Immerse a clean cloth in this solution and apply it to the hot spot for minutes. Do this four times a day.

The New Remedy – Apple Cider Vinegar

Apple cider vinegar has anti-bacterial and anti-inflammatory properties and is used in quite a few other home remedies such as ear cleaning, of which it is highly effective. (I know because I recently used it to clean my dogs ears and it did a heck of a job – better than any ear cleaning solution I have ever purchased). It also houses potassium in high form, which is good for older pets, and can be consumed orally to boost the immune system. I use apple cider vinegar in my dog’s food on a daily basis.

Soak a cloth in apple cider vinegar and apply it directly to the hot spot 4 times a day.

Some California Injury Victims Receive Less Compensation Because of Their Health Insurance

People who suffer traumatic brain injuries, spinal injuries or other serious injuries as the result of the negligence of a third party must receive appropriate medical care. Victims who do not have health insurance typically obtain medical care on a lien basis, and their liens are resolved after settlement or a judgment is rendered in the case. To determine damages based on medical expenses in these cases the judge or jury must determine whether the treatments were necessary and the charges were reasonable. The entire medical bill is presented to the fact-finder (usually a jury) at the trial.

This procedure is very different from that of cases involving medical expenses submitted to a health insurer. A victim whose medical bills are paid by an insurance carrier is only responsible for a co-payment or the deductible. And, insurance carriers almost never pay medical bills in full. Usually, a large portion of the medical bill is discounted because of rate reductions negotiated by the insurance company.

As the law stands victims in California personal injury lawsuits who have health insurance receive less compensation than victims who lack insurance. How do the courts assess damages for a personal injury victim who pays medical bills with private health insurance. The cases that deal with this issue have emphasized the public policy of not penalizing victims who have medical insurance. Less emphasis has been placed on cases in which medical bills are written off or deeply discounted due to contracts between insurers and healthcare providers. The issue is important to victims in cases of traumatic brain injuries, spinal injuries, and other serious injuries in which the treatment is usually long and very expensive.

The California Supreme Court has ruled that medical bills paid by health insurance should be included in the evidence given to the jury. The Court has stated that a victim should benefit from purchasing health insurance. A victim in a personal injury case can put forth evidence of all medical bills charged regardless of how the bills were paid. Those bills provide the jury with evidence of the amount of damages the victim should receive to compensate him for his bills. The bills also aid the jury as they assess the injuries of the victim. Presentation of the total bills assists a jury or judge in determining how much to award a victim for his or her pain and suffering.

But, following a trial at which the full medical bill are presented the defense may request a hearing to reduce the amount of the damages awarded to compensate the victim for medical bills to reflect the write offs or reductions due to health insurance contracts with medical providers.

The solution the courts have come up with is eliminating from damages the medical bills that have been written off. This is at odds with the original rationale for allowing the medical bills of insured victims to be introduced at trial. The rule was designed to prevent the negligent party from benefiting from the decision of the victim to purchase insurance. The idea was to encourage victims to have insurance. Reducing the recovery of the victim because of write offs or insurance adjustments benefits the negligent party. If the victim had no insurance the negligent party would be responsible for the full cost of the treatment. It seems logical that since the victim paid the premiums for the insurance, he or she should receive the benefit of any write-offs or contract reductions.
